It’s often only when we’re facing a medical emergency, a legal decision, or an unexpected loss that we realize how essential one notarized document can be. Whether it’s a Power of Attorney, a Living Will, or a Healthcare Surrogate form, that single signature can mean the difference between clarity and confusion—between peace and panic.

Key Documents That Often Require Notarization:
- Power of Attorney – Grants someone authority to make decisions for you while you are still living.
- Two things to know:
- It becomes invalid upon the signer’s death.
- The signer must be of sound mind and acting voluntarily.
As a notary, I am legally required to decline notarization if coercion or lack of understanding is suspected.
- Advance Directives / Living Wills – Outline your healthcare preferences clearly and legally.
- Healthcare Surrogate Designations – Assign someone to speak for you in medical decisions.
- Guardianship or Custody Documents – Often necessary when caring for minors or vulnerable adults.
Before We Meet — What You’ll Need:
- A current, valid photo ID (must be government-issued; acceptable forms include a driver’s license, passport, state ID, or military ID)
- All signers present and willing – No one should be under pressure or unsure of what they’re signing
- Any required witnesses – Especially for medical or estate-related documents

- A complete document – All fields filled in except signature lines
